Remedies for an early sore throat

Honey, black seed oil, lemon, vinegar, propolis, mushrooms: Professor Akim Bouterra shares his early sore throat kit above. This guide starts with the question that matters most, viral or bacterial, and the signs that call for a test; it places each remedy against what the trials have shown, gives the recipes and doses, and what also works on swollen tonsils, before explaining when antibiotics become necessary.

The essentials in 30 seconds

Eight out of ten sore throats in adults are viral and clear up on their own within a week; the professor's remedies make them more bearable. Honey soothes the throat and cough (Cochrane review); propolis spray reduced the duration of symptoms in a randomized trial; a black seed extract outperformed the placebo for sore throat in another. A bacterial strep throat, more common in children aged 3 to 15, is recognized by high fever, painful lymph nodes, the absence of a cough, and white spots, and is confirmed in thirty seconds with an in-office test: in that case, antibiotics protect against complications (rheumatic fever, kidney damage), and the remedies work alongside them. White spots alone don't mean it's bacterial: viral sore throats have them too.

A sore throat shows up as white spots at the back of the throat, and to avoid complications it should be treated as quickly as possible.

As a first step, we recommend taking one tablespoon of honey combined with a small teaspoon of black seed oil, lemon, and a bit of vinegar.

It is also essential to use propolis, which has antibiotic, antiviral, and anti-infective properties. It can be used in its raw form, but it is best taken as a syrup, either on its own or combined with honey.

In addition, taking an adaptogen such as a Chinese mushroom like cordyceps, reishi, or maitake will help boost the immune system. These are available in capsule or vial form.

The professor says you must "treat it as quickly as possible to avoid complications": this is true for strep throat, which is why white spots with fever in a child should be shown to a doctor on the first or second day. In an adult without fever or swollen glands, you can start with the remedies and wait three days.

The professor's toolkit, and what the trials say

  • Honey. The prophetic remedy for the throat: it coats, soothes the pain and the cough, and its antibacterial action is real on contact. A meta-analysis of 14 trials found an effect of honey on the severity and duration of upper respiratory infection symptoms (BMJ Evid Based Med, 2021).
  • Black seed. A randomized, double-blind trial in patients with acute sore throat: a black seed extract combined with another plant reduced pain and difficulty swallowing compared to placebo over seven days (Int J Clin Pharmacol Ther, 2008).
  • Propolis. "Absolutely," says the professor, and he has a trial to back it up: an oral spray of standardized propolis cleared uncomplicated upper respiratory infection symptoms within three days in most patients, versus five without it (Phytomedicine, 2021).
  • Lemon and vinegar. Acidic, they cleanse and tighten; a folk remedy without a trial of its own, well tolerated in small amounts in honey.
  • Mushrooms (cordyceps, reishi, maitake). Immunomodulators from Chinese tradition, useful for prevention in those who get recurring sore throats; no evidence of an effect on an active sore throat.
  • Indian costus, which the professor mentions elsewhere: it is the remedy for sore throat named in the hadith itself ("it is inhaled through the nose against sore throat"); see costus.

The recipes, from day one

  1. The professor's blend: one tablespoon of honey, a small teaspoon of black seed oil, the juice of a quarter lemon, and a teaspoon of apple cider vinegar, mixed together; let it dissolve slowly in the throat, three times a day. For children over one year old, honey and lemon only.
  2. The salt gargle: half a teaspoon of salt in a glass of warm water, 30 seconds, four times a day; it cleanses the tonsils and reduces swelling.
  3. Propolis: as a throat spray (the best form, according to the trial), in lozenges, or in syrup with honey, every two to three hours during the first days.
  4. Costus through the nose, following the hadith: a pinch in a drop of olive oil at the edge of the nostrils, or as an inhalation, twice a day.
  5. Honey in warm water morning and evening, and a thyme or ginger herbal tea with honey during the day; drink plenty of fluids, warm rather than cold.
  6. Rest, a humidified room, no tobacco or smoke, sparing the voice.

Swollen tonsils: what brings the swelling down

The tonsils swell because they are working: they filter out the infection. What helps: salt gargles and honey applied directly, alternating cold (an ice pop or cool water soothes the pain), a pain reliever (acetaminophen; ibuprofen only on advice, never when a bacterial cause is suspected and untreated), and time. Tonsils that stay enlarged after recovery, in a child who snores or breathes through the mouth, should be shown to an ENT specialist. Abundant pus on one side alone with inability to open the mouth is an abscess: an emergency.

Contraindications and precautions

No honey before one year of age; no black seed oil by mouth before age three or during pregnancy; no propolis for those allergic to bee products (it causes reactions more often than honey does) or for asthmatics without a cautious trial. See a doctor the same day if fever goes above 102.2 °F, if you can no longer swallow your own saliva, if the mouth opens poorly, if only one side is swollen, if spots appear on the skin, or for any feverish child with white spots; and by the third day if nothing improves. A confirmed strep throat is treated with antibiotics through to the end: stopping on the second day because "it feels better" exposes to the very complications it was meant to prevent. The professor's remedies don't oppose the antibiotic, they complement it.
